Last Entry showing in Sign up Form

OK, does this happen right after submitting the signup? Or after you go back to the signup page or reload the signup page?

After I submit a message displays to user that it was accepted and then when I go back to signup record is still there. Website is https://www.newcastleac.org. Last test record is showing there now.

When I submit form it does what it is supposed to do and the record is saved and is listed in Admin. It just does not reset the fields to blank.

Curious. Do you have default values set for any of those fields?

OK, don’t set your file uploads location to be inside the plugin directory. The default is: wp-content/uploads/participants-database/ and unless you have a specific reason to have it elsewhere, this is a good place to put it.

Try that and see how it works.

Changed location and get error
There was an error uploading the file. This could be a problem with permissions on the uploads directory.

Yes, you have a permissions issue…and it looks like you still need to get your file upload location setting right….you’re still trying to put it into the plugins directory.

The permissions issue you will have to go into your control panel or FTP client and make sure the wp-content/uploads directory is writable. This has to do with your site hosting setup.

Ok created uploads directory and changed permissions and records have now been uploaded. Still can’t edit them though – back to original problem

this problem is however a lot less important than the problem of the signup form not resetting

OK I see what’s happening with your signup fields. You need to uncheck “persistent” on any fields you have in your signup form. I’m going to fix this so it won’t matter, but this will fix it for you right now.
