Latest Update Causes Fields to be added to User Profile

    After updating to Version 4.0.3 on a test site, the following fields now appear in the user profile that were not there before:
    Street Address
    City
    Country
    State/Province (this field has dashes as the default value and all of the dropdown items are “Array”
    Zip / Postal Code
    Phone

    How can I remove these fields?
