    This is a bit out of the scope of the plugin. You can hide the form with css

    #ugc-media-form { display: none;}

    And then display it with jQuery when “Agree” button is clicked.

    $('#agree').click( function( e ) {
  e.preventDefault();
  $('#ugc-media-form').show();
});
