Lightbox Email plug in

I am looking for a Lightbox plugin that would require the user to enter their name and Email address to view a page. The page would be greyed out in the background and unusable until they provided that information. Does this exist already or is this something I would have to pay to have developed.

I have not found any plugs that looked like they would require information before allowing the user to proceed.
