To add a form you need to use a form plugin such as “Contact Form 7”, etc.

I do have mail chimp, can I use that?

MailChimp.com already provides pop-up form so you do not need our plugin for it. If you plan to use our plugin called “MailChimp for WordPress – https://www.mc4wp.com/”, then you can use it with Boxzilla.
