Link has disappeared when click on Download Now button

Hi!

The download now button was working. I made a change to the list and it said the page had disappeared.

I just went in the plug in, changed the unique list ID, and it still says the page has disappeared. So folks still cannot sign up to get the report.

Can you help me?
