Linking Custom field Value to category with the same name

Hi,

I have custom fields that are used for the address in a custom post. These fields are “City”,”County/State” ,”Postcode”. Say the value I put in “County” was “Central London”. If I was to make a customized taxonomy with the same name ( central london), is there a way how I can link the two.?

So that when I select “Central London” in the “County” field and check the post, it becomes a clickable link to the category with he same name.

I want it to be like this because there are under 10 counties in total, that I will need to add to my website. However, I will have 100s of posts with that could select any one of them.

I have a vision that the user of my site will see the address, click on the county and then see other posts from the same county.

I am using Advanced Custom fields for my custom fields.
