list for regional contacts multiple addresses

  • is there a simple plugin or way to list multiple contacts around the usa so they pick a state or country from a drop down menu right now I have a customer who wants to be able to do this : https://www.imperialrange.com/sales.php

    on there wordpress page right now i have all the contacts on one page in tables.

    just wondering if there is a plugin that could simplify this
