I already have this plugin on 2 other sites and it works fine.

If the plugin is setup and working then all I can say is that it is a theme issue. I would suggest looking into the themes template files for WooCommerce, mainly the checkout area and compare with the default templates located in the core of the WooCommerce plugin. It’s possible that there is an action hook outdated or has been removed from the theme templates.

Not sure how else I can help so I hope my advice helps you solve this issue.
