Login Form Not Working

How/where do you give the visitor the place to enter a username and password in the free version?

My site is built on Divi and runs the Divi them (in case those are problems). I was successfully logging folks in using LoginWP (so I know the login process works) but would like some of the other features UM offers.

I deactivated LoginWP and several other plugins and activated UM. I installed the login form on a page. But the form does not have a place for the username or password — just “Login” and “Register” options. When I click “Login,” the bottom half of the screen displays a red box saying “Please enter your password” (where?) and the top has a list of 6 error messages pretty much saying there is no user password identified.
