login page error in Chrome

For some reason my login page is now populating “undefined” in my username field and then redirects to a page that asks for a math problem verification. This is stuck in a loop. You solve the math problem it pops back to the login page and the whole process starts over. This is only in Chrome, Firefox seems to be fine. My website seems to be fine. I cleared all cache/passwords/autofill and it is still doing it. Any thoughts about why and how I might fix this? I really prefer to use Chrome.
