login page not showing up correctly

I have used this plugin on one site and it worked fine. Just using it to protect one page with a login only.
https://www.whatsahead.com/supplier-toolkitHowever, I am creating a revised site and wanted to use the same plugin the same way, but it is appearing totally different. I believe I have the same settings in place. I do not want a registration page, just a login page with username and password and copy above.
