Looking for a form database plugin, help! :)

What I’m looking for: a form submission plugin that will allow the users of my community to submit a form to me for approval. The form should remain editable by the user until I (admin) lock it. The form would always be associated with the users login details and it would be stored in a database until deleted by either the user that created it or an administrator. The form should always be viewable to the creator and the admin only.

I have tried a lot of different forms and I can’t find one that does these things.
