Looking for a form / survey plug in

Hello people!

I’ve been looking at different plugins, but I can’t quite find exactly what I’m looking for.

Basically I’m helping a friend build a very small website where he could ask his clients to go on and answer various questions so he can evaluate the cost to produce their Tax Returns.

So I’m looking for a plugin that would

• Allow me to create a question form
• Ideally this question form would be “reactive”, by that I mean that – for example – if the answer to a question is YES then it would lead to “sub questions” while if the answer is NO then skip to the next question. A little like a survey, somehow
• The answers would be received by the webmaster. Maybe by email, ideally in a database (SQL)
• Ideally the answerer would get a confirmation by email

Contact Form 7 was the closest to what I’m looking for however it lacks some features (as described above)

A good example of what I’m looking for would simply be the process when you go through a survey (i.e.: Survey Monkey) like I said above.

Is there any plugin that could le me do something like that?
