Looking for customer portal plugin

Hi,

I’m hoping someone can help me. We have a photo booth business and we’re looking for a plugin that does the following:

-Allow clients to login.
-Clients enter in all their information and can read and agree to terms and conditions
-We’d like to give clients a different selection of design templates to pick from
-Allow them to put in any additional information they’d like displayed
– We’d like this information to be sent to us, or a notification sent to us so we can see the information entered.
– We’d like to then be able to upload their design option with any changes requested to the same portal for them to login and view.

Does anyone know of a plugin that will allow us to do this?
