Looking for file uploader plugin

Hello, I’m new to WordPress and am looking for a file upload widget/plugin with the following functionality:

1.) Supports tiff, pdf, gif, png and psd files, or allows the admin to choose which files types can be uploaded

2.) Supports multiple file uploads

3.) Requires the user to fill in customizable text boxes (similar to a contact form) before the files can be uploaded

4.) Sends an email to a specified address upon successful upload

Any ideas would be greatly appreciated!!
