Looking for plugin – or developer – to make an order form like one in link

I am setting up an online store to take orders for archiving services – scanning, digitizing etc. ScanCafe, which does the same thing, has an amazing form that allows the customer to add values to a rolling spreadsheet and then submit an order. I would like to know how they did it? Is there a wordpress plugin that will get me in the same ballpark? And if I need some serious tweeking, where could I go for developer help? I don’t want a big contract for someone to make me a custom form; just someone who can get me over my initial ignorance.
