Looking for State by State Location Listing

So what I am looking for is to have a plugin or hack php that will allow me/us to list “locations” or “affiliates” by state search.

So in other words I add a location (diner, bar, rest stop) by state and a person can simply click or via a pulldown select the state to see if there are any locations in that state. I simply want a listing. No Google maps etc.

Make sense? Is there anything out there.
