Looking for WP Form Plug-in

Hi,

I have been searching high and low, but can’t seem to find a form plug-in for wordpress that does the following:

The user selects from a series of drop down menus that change depending on the options that are selected, and ultimately displays a specific page based on the selections.

For example: The user selects their Country from the first drop-down, then based on that, the cities for this country are displayed on the second drop-down, and based on this, the specific city page is displayed.
Country>City>City Page Displayed.

Any ideas about a plug-in that could do this? I think it is a very basic form..
