Lookup not working properly – Formidable

Been troubleshooting with Formidable but haven’t found a solution.

I have a form that requires a user to enter their email address (Users are logged out and there is no requirement to log in). Upon this, it fetches the personal information from a different form to display it for review. Entering my email address, [Moderated: Email address redacted], should populate all read-only fields below it (First Name, Last name, Address, City, State, Zip Code). First, it’s very inconsistent. Not all fields populate and it appears that the information is incomplete when it is not. Second, since I’m just testing, I will call the page again, enter the same email address and a different set of fields may populate but, again, not all populate.
Everything works perfectly when I’m logged in as admin or as user. I’m doing all testing in Production.

Here’s the troubleshooting I’ve done to no avail (I’m doing the troubleshooting in staging):
Synced production to staging
Ensured WP and Formidable are latest versions.
Disabled all plugins and custom PHP
changed theme back to default 2017 theme

Formidable replicated the form and ran it on their server and said it worked fine, that I should refer to GoDaddy to troubleshoot. GD said that I should reach out to you for a resolution.

I’m starting to feel like a ping pong ball.
