Lost historical button counts and current twitter counts

Hey Aakash,

Thanks for the great plugin.

We recently moved our blog from shared hosting to a VPS and in the process we lost all of our previous Facebook likes, Tweet counts and Google+ counts (StumbleUpon counts are working Ok).

For example if you visit: https://wanderlusters.co.uk/travel-photo-roulette-natural-world/ the three counters are showing zero. However if you enter that URL in https://developers.facebook.com/docs/reference/plugins/like/ it says 96 people like the link. The Tweet count was also around 200 and Google+ around 10.

Any new posts that we have made since the site relocation such as: https://wanderlusters.co.uk/wai-o-tapu-thermal-wonderland-in-rotorua/ seem to work Ok apart from the Tweet count which continues to stay at zero even if the button is clicked.

Any help to fix these issues would be hugely appreciated.
