lost my survey

Hi

my survey doesn’t show anymore on my website, it just disappeared, only the shortcode is displayed.

And it’s not listed anymore in the admin, in the wpss surveys list.

I have checked in mysql databases and the survey is there.

Can you help me retrieve it please ?
