Lost the fields and sign up button

Just checking in to see if this was resolved and you erased the original post, or if something in the www.ads-software.com forum went bonkers.

If the issue is still occurring, please include a link to your site. Not quite sure if this applies, but the form fields won’t display if the form action field is blank (to prevent folks from trying to sign up before you have a chance to set it up).
