Mac OS/Chrome Slider Not Fully Indented

Hi Guys
We have the latest version (1.52) of the plugin running on the Avada (4.03) theme it is working perfectly across the board with one exception, Mac OS and in Chrome. Roughly 75 px of the form div is visible, meaning it is not completely indented off screen. Any help would be greatly appreciated.
