MachForm not showing on page

I have setup the shortcode using the Machform URL/Location: https://www.sethgoochtraining.com/portal/ and created a new page called portal. I pasted the shortcode on the portal page, however, it’s not showing. I can’t figure out what I did wrong. I might be missing something simple, but drawing a blank. Can you assist me with this?
