Mailchimp For WordPress does not work properly

Hi, I have designed a form in General Forms, Create Forms, Design it, in mailchimp and I’m not sure if I am doing it correctly but the form I design does not come across to my WordPress plugin, only the basics; email address, first name, last name, submit button and the boxes, that is all that comes across. Is there something I have to do to make it happen????? Thanks if anyone can help, Im using the Vantage theme.
