MailChimp List Subscribe Form: 1.2.5 not working after WP 3.2.1 upgrade

Any ideas or suggestions for troubleshooting?
Emails are not being sent. Also, success/error messages are not being displayed. For example, there is no message when entering duplicate email addresses… and there is no ‘success’ message after submitting a new email.
