Mailpoet Signup Tag does not show

I’m trying to add the mailpoet signup tag to a contact form but when selecting the tag generation in the contact form, it shows only a title saying “Mailpoet Signup” without the necessary fields to actually generate the tag.https://www.ads-software.com/plugins/mailpoet-contact-form-7/
