Maintenance mode sometimes not active

We have a site with your plugin installed, and our client viewed the site during the night 04.59 local time, and could see the content. The site shouldn’t have left maintenance mode, as far as I can discern. Do you know what the problem might be?
We have had this problem before, about a year ago on another site.
