Make and Share a database to another page

I would like to have a page where a user can fill out a form and upload an image, submit that to a database and when referenced on another page (not the same page as the form) the information of all shows up.

Ex. A group of boys and girls with different names fill out a simple form (Sex, Name, Upload Image). On the page boys, all subjects are shown and can be organized alphabetically. I would like the image to be next to their name. I would also want this to be an interactive list, where I could click on their name or photo and receive more information about the individual.

I was just looking for a basic example, if you have a better way of describing feel free.
