Mandatory Name and Email fields have disappeared from my form

Hello. I think this is a very cool plugin! I have run into an issue, however. I created my comment form. The Name and Email are mandatory, so I added three more fields. At first everything showed perfectly. I even submitted a few test comments. Now, the Name and Email fields have disappeared from my form. They are there if I edit my form, but do not show up on my page. This is very strange. Does anyone have an idea what is causing this?
