Map Icon Not Appearing in WYSIWYG Editor, TinyMCE Is Enabled

The map icon does not appear in post WYSIWIG/visual editor, even though I have saved a shortcode and TinyMCE button is enabled in plugin settings.
I’m using WP4.0 with Foghorn theme.
