Media Button disappeared

The External Media plug-in button disappeared and I have not been able to retrieve it. I have deactivated and reactivated as well as checked for other plug-in conflicts.

This occurred after I was attempting to change some settings on my home-page header. I made header changes in the sylesheet files. I do not believe they are related but it could be since I am not familiar with the plug-in.
