Media Categories option missing

I have version 2.0.4.8 of the plugin installed on a WordPress 4.3.1 install. When I click the “Add Media” button when writing a new post, I don’t have the “Media Categories” option on the Insert Media pop-up any longer. The only way I can assign a category to my images is to go into the Library and do it there. Here’s a screenshot of how my Insert Media pop-up looks. Any idea why the Media Categories option is missing and how I can get it back?
