media replace not working?

Hi
I have WP 4.5.2 and latest version of plugin and am trying to update image files using ‘replace’ but when I do the old image is till there. I have tried simply deleting the old image a then re-uploading it and the same thing is happening ??
Any ideas? Many thanks
