Media uploader creating hundreds of attachment entries

I changed site from multisite to single few weeks ago and recently I’m discovering several issues and I’m not sure weather or not the issues existed because of this change.

Media gallery never completes upload and there are hundreds of entries of attachment of one single upload attempt (which never completed).

Steps checked
.htaccess
wp-config.php
plugins checked
re-installed WP

any help would be helpful
