I stand to be corrected but first name and last name are not part of PMPro default registration fields. You would need to add it yourself and it is not difficult.

Register Helper does not work. When activated it gives an error.

I disabled all plugins to make sure there were no conflicts and it still does not work.

Any other ideas on how I might be able to add those fields?

This is definitely an issue and should not happen, I tested this on my end and I am unable to recreate the issue.
