Minor issue with “Comments Form” on website

Greetings.

I am having a little problem with the “Comments” form on my website.

I have modified settings to include comments for each and every post and page.

It used to work well. The comments form was displaying with only 3 fields: Name, Email, and Comment.

And, today, all of a sudden, the pages are showing a rather lopsided comments-form. Which now includes an extra field for “website”. No idea where this came from.

I’m not sure where the error is. Perhaps, I should choose another format for my Comments-Form?

In which case, where can I do that?
