Missing default cover images

I have an issue where I have 4 html players on the frontpage. Been this way for a couple years.

For some odd reason in the last few months only 1 of the players is actually displaying the default cover image and the one that is displaying won’t update when I update the image. It has nothing to do with caching.

Any ideas?
