Missing Descrition box

In our support form under new ticket we have all fields except the Description text field. Clients can write their subject and select category and priority but they can not write their comments.
How can we fix this. We have a 100 support tickets to answer and don’t want to loose them all. The page is https://trulyheal.com/faq/
I am happy to pay for support but I need to resolve this quickly.
