missing modal, stray code & how to remove comments issues

I’m rolling three issues into one. My apologies if this causes confusion.
I decided to try Tubelar because the sample shows it uses a modal window set up close to what I want to use. The problem is that all I get is my form on a page. The best solution would be to just have the modal and leave the pages out of the equation. At the very least I’d like to recreate the semi-transparent dialog box, preferably on top of my regular home page, rather than one I have no use for, turning it into a soft squeeze.
The second problem is that it shows up looking like a blog post with the comments disabled, announcing “no comments.” Since it doesn’t show up as either a post or page, how do I remove date, author and the comment status?
The final problem is some stray code showing at the top of the page.

is at the top. I can’t rule out the possibility that there is code at the bottom because it would blend into the footer. The temporary location for this is . Since it’s published it should be visible. thank you
