More weirdness: no records error, duplicate check not working, swapped records

Thank you for bearing with me. Your responsiveness as a plugin developer is unusual – much better than some paid plugins – and I just left a donation in appreciation equivalent to what I would pay for a premium plugin, which yours is.
That said, things seem to be broken again, and I am hoping you can take a look. Before I get into weirdness, here is what I’ve don’t recently to try to troubleshoot based on other support threads:
– turned off apache mod_security for this domain in my hosting account (don’t know if that is still an issue). The site is on Dreamhost
– installed php error log plugin for wp dashboard (no errors yet but I just installed it
– checked that my daily backups are being made so I can roll back if needed
– tested on Chrome and IE
– deactivate and reactivate pluginHere’s three types of stupifying weirdness going on (in addition to problem with thank-you page text insertion upon request-link submit that I posted as a separate topic). This is a long (many field groups) multi-page form.
1) Record Not Found: On Signup page (https://doyoncandidate.iialaska.com/candidate-form//candidate-form/), when you fill it in and hit Save button at bottom, sometimes it a) fails to do anything; b) takes you to page 2 (https://doyoncandidate.iialaska.com/occupation/) with a “No Record Found message; or c) submits OK and takes you to page 2. The shortcode on both the signup and pdb record page seem correct.
2) Duplicate Record Check: The Email field is set as the duplicate record field to check (and I’ve double/triple-checked this), but the site is no longer preventing two records with the same email from being submitted. When this happens, no error message is shown. I could provide screen shots of both the List Partic. screen and the Signup Form settings page to verify.
3 – and most worrying) One of our user testers completed entering her information, when she hit submit, another users info came up instead of hers. They work in two different cities, 6 hours apart, so I know they weren’t using the same computer. In my own testing I have had the wrong record show up when clicking on a private link from the link participants screen.
Please let me know if you have any ideas. Also if you would like access to the WP backend. I have suspended additional user testing until we have looked at what is going on.
