    Note: this appears to work fine so long as the plugin is not network activated.

    I found that if you network activate the plugin, then go to the Widgets section, you can add a widget to the sidebar that lets you turn this on or off on demand. You have to explicitly turn it on for the site in order to then see the form on each comment.
