Multi-page Gravity Form Breaks Plugin

I have a multi-page Gravity Form and am trying to get colorbox working on all images in the form. Images on the first page of the form are opening correctly, and I can even scroll through all images from the modal window. However, when you advance to a subsequent page, images on those pages do not trigger the colorbox effect. The page url does not change and it appears that the javascript and css are still loaded on the page. See it in action at https://www.pattersonmfg.com/controlrfq
