Multi Step Conditional Logic Plugin

Hi,

Hopefully someone can help, I am looking for a plugin that can achieve a Multi Step Conditional Logic synario, if you visit this page and scroll down to and click on Book Now under Children’s Swimming Lessons you can fill out a Multi Page form where answering yes takes you to the next Question but answering No just shows you some content which includes a button. The form also contains a back button.

I have been trrying to achieve this with a variety of different plugins but none are either cutting the mustard or Im missing something.

Any help would be gratefully appreciated.
