    I’m trying to build multi-tier (sub-set) check box for my form. Here’s an example:

    (title): Bike Make
    (field choice 1:): Salsa
    (field choice 2:): Spot
    (field choice 3:): Surely

    If “Salsa” is selected, then a sub-set of options become available…

    (title): Salsa Models
    (sub field 1:): Vaya
    (sub field 2:): Metro
    (sub field 3:): Fargo

    The if the Salsa “Vaya” model is selected, another sub-set becomes available:

    (title): Salsa Vaya available colors
    (sub field 1:): blue
    (sub field 2:): white
    (sub field 3:): red

    If not plugin is available, is there a way to construct this in HTML5?

    I found a solution after all. the Gravity Forms plugin has “conditional logic” settings that has allowed me to do exactly what I want.
