Multiple Acceptance Checkboxes for Constant Contact

I have been successful in creating contact forms that allow me to add a user to a single Constant Contact email list or multiple. However I am wondering if there is a way to setup a single form with multiple acceptance checkboxes that link to multiple lists, thereby giving the user the ability to check off the 2 or 3 lists that they would like to join.
