Multiple choice in form

Thank you for contacting us.

You won’t be able to create a form with the specific select box as mentioned using Hustle. However, you can do this with our plugin Forminator so that you can create the form just like on the reference page(using select fields). Then, once you have the form, you would copy the short code and paste it inside the Hustle popup informational editor so that when the popup shows, the form will show with it.
