Hreflang on multiple domains and installations

I have a company that are in multiple countrys and every country have their own domain.
I would like to be able to link a specific page to all the other countrys version of that page.
Also on every domain there are a english version, so I would like to have multiple hreflang, for exemple:
sv-se
nb-no
fi-fi
da-dk
lv-lv
en-se
en-no
en-dk
en-lv
sv-fi

Does somebody know how to fix that?
