Multiple form instances for multiple quantities on product page

  • I have a form on a product page that needs to be filled out per quantity ordered.

    eg.
    First name
    Last name
    Date of birth

    If a customer orders a quantity of 3 for the product, I need 3 instances of that form to be entered.
    Currently, I’ve only managed to configure the form on the product page for one instance by default.

    Is this type of functionality achievable?
